PDC Health Hub

PDC Health hub is a collaboration of allied health professionals including Counsellors, Diabetes Educators, Dietitians, Exercise Physiologists, Physiotherapists, Podiatrists and Psychologists. The team at PDC Health Hub Allied Health Team work together in a single organisation, a unique quality that is not offered by any other community-based health organisation.

The PDC Health Hub team pride themselves on the innovative approach offered to clients through a focus on collaboration, communication and shared care. This has provided less repetition and greater outcomes for clients attending PDC Health Hub.

Perth Diabetes Care

Perth Diabetes Care is a major hub of PDC Health Hub. Starting in 2015, Perth Diabetes Care has grown to be the largest private Diabetes Education provider in WA.

Perth Diabetes Care carries the innovation of PDC Health Hub, utilising the collaboration of all the allied health team to provide an individualised service that is completely focussed on achieving your goals and targets in your diabetes management.

Perth Diabetes Care has an award winning team that can assist with all types of Diabetes including Type 1 Diabetes (T1D), Type 2 Diabetes (T2D), LADA, MODY, Gestational Diabetes and Diabetes in Pregnancy.

PDC Allied Health

As part of the collaboration, PDC Allied Health offers a collaborative allied health team to assist with wellness, weight loss and chronic conditions.

The PDC Allied Health team consisting of Counsellors, Dietitians, Exercise Physiologists, Physiotherapists, Podiatrists and Psychologists can assist with all chronic conditions such as Arthritis, Cardiovascular (Heart) disease, chronic pain, high blood pressure, high cholesterol, weight management (weight loss and weight gain), as well as overall health, wellness and prevention of disease.

PDC Fitness Hub

The PDC Fitness Hub is an Exercise Physiologist and Physiotherapist run boutique gym located at PDC Health Hub Headquarters in East Victoria Park. Our team of Physiotherapists and Exercise Physiologists work together to assist people to overcome and prevent acute and chronic health conditions, injuries and promote health through education, exercise prescription and lifestyle advice.

PDC Fitness Hub offers a range of services including one-on-one consults and exercise sessions through to to semi-private group sessions. At PDC Fitness Hub you will never be unsupervised - our small class sizes mean that you will always have plenty of supervision and guidance from our team in reaching your health and fitness goals - in a fun, safe and supportive environment.

PDC Fitness Hub is fully equipped with cardio and resistance training equipment and exercise prescription software. Our PDC Fitness Hub Exercise Physiologists and Physiotherapists look forward to helping you live a happier, healthier, stronger, fitter and pain free life.
